正常成人每天合成的免疫球蛋白為2～5g[1]。但是在受到感染以后，其的合成量可增加7倍左右[2]。因此，對Ig的測定可以作為檢測多種疾病的依據(jù)，同時在臨床上，還將其作為臨床診斷和觀察以及預(yù)后的重要觀察指標(biāo)。然而，對血清Ig的檢測結(jié)果也同時受到多種因素的影響，比如檢測儀器；標(biāo)本處理；操作技術(shù)；實驗試劑等。在同一實驗室的不同批次或不同實驗室中對其進行檢測，也會出現(xiàn)一定的差異。但是，在國內(nèi)對血清標(biāo)本的放置時間和保存溫度對IG的檢測結(jié)果存在的影響，報道的非常少。本文采用全自動生化檢測儀器對不同溫度和時間對血清進行保存，并對Ig進行檢測，主要是探討臨床上保存血清的最佳條件。以下是詳細(xì)報道。

1 資料和方法

1.1 一般資料 我校在2012年6月～2013年10月期間，一共選擇了200名健康的體檢學(xué)生。都是男性，年齡在22～30歲之間，平均年齡為（27.5±2.0）歲。他們都沒有感染性和系統(tǒng)性的疾病，同時沒有出現(xiàn)肝腎和心血管等疾病。

1.2 方法

使用干燥的試管對他們進行靜脈抽血，大約為5ml。在室溫下放置1h，使用3500rpm進行離心10min，最后去最上層的血清，分開裝到5根試管中。分別放置到室溫和4℃以及-20℃的環(huán)境中進行保存。在室溫下進行保存的血清，在保存2h后，對其進行檢測IgA、IgG、IgM。保存在4℃和-20℃的血清，分別在第3d和第6d進行檢測。IgA、IgG、IgM的定量測定試劑盒由上?？迫A生物工程股份有限公司生產(chǎn)。用Hitachi 7180全自動生化分析儀（日立公司生產(chǎn)）進行免疫比濁法測定，所有操作均嚴(yán)格按照試劑盒說明書進行[3]。

1.3 統(tǒng)計學(xué)方法 對所有的數(shù)據(jù)都使用均數(shù)±標(biāo)準(zhǔn)差（x±s）來進行表示[4]。并使用SAS15.0軟件進行統(tǒng)計和分析。并采X2檢驗。差異顯著具有統(tǒng)計學(xué)意義（P

3 討論

免疫比濁法是目前臨床上測定血清IgA、IgG和IgM的主要方法，包括透射比濁法和散射比濁法。兩種方法的測定原理和儀器不同，其結(jié)果有所差異。透射比濁法主要檢測抗原抗體復(fù)合物所形成的濁度，用全自動生化分析儀檢測；散射比濁法是從不同角度測量抗原抗體復(fù)合物微粒的散射光強度和濁度的變化，用蛋白分析儀檢測。一般認(rèn)為散射比濁法的靈敏度高、重復(fù)性好，但國內(nèi)研究顯示散射比濁法和透射比濁法準(zhǔn)確性、精密度均比較好。但散射和濁法和透射比濁法測定Ig的結(jié)果，發(fā)現(xiàn)偏差均在可接受范圍，變異系數(shù)均小于3%，說明二者均為IgG、IgA、IgM的可靠測定方法。全自動生化分析儀的應(yīng)用，使Ig的檢測速度大為加快。如果使用性能較好的全自動生化分析儀及相應(yīng)的配套試劑，就能保證IgA、IgG和IgM透射比濁法測定結(jié)果與散射比濁法一致。日立7180全自動生化分析儀具有處理能力大、測試速度高、標(biāo)本用量小等優(yōu)點，在精密度、線性、準(zhǔn)確度等方面性能良好。

血清標(biāo)本的分離與保存、檢測儀器和試劑、操作技術(shù)等因素均對Ig檢測結(jié)果有影響，在檢測過程中必須做好各個環(huán)節(jié)的質(zhì)量保證措施，減少或消除檢驗誤差。用全自動生化分析儀測定血清樣品，新鮮標(biāo)本較好。由于免疫比濁法測定的是溶液濁度，故脂血、溶血標(biāo)本對測定結(jié)果影響很大。適宜的溫度可加快抗原與抗體結(jié)合，從而加速了濁度反應(yīng)。在0～56℃之間，溫度越低，反應(yīng)越慢，但反應(yīng)生成的抗原抗體復(fù)合物結(jié)合致密、牢固，易于觀察；溫度越高，反應(yīng)越快，但反應(yīng)生成的抗原抗體復(fù)合物不穩(wěn)定，易解離。溫度過高（>56℃）可能引起抗血清球蛋白變性及某些血清球蛋白破壞。因此，多數(shù)抗原抗體反應(yīng)以37℃為宜。

經(jīng)過實驗表明，在4℃下對血清進行保存3d，IgA的含量出現(xiàn)了明顯性的變化。然而IgG在6d時也出現(xiàn)了很明顯的升高情況[6]。但是IgM卻沒有出現(xiàn)變化。在-20℃條件下對血清進行保存3～6d時，IgA和IgG以及IgM的含量都沒有出現(xiàn)明顯性的變化。在4℃時對血清進行保存時，IgA和IgG的含量升高，然而IgM的含量卻沒有變化，主要是因為血清中的IgA、IgG大多是單體。IgA和IgG的分子肽鏈相對于IgM來說，更容易被血清中的蛋白酶水解掉各種的片段。然而這些被水解的片段又會與試劑中的相應(yīng)的抗體進行結(jié)合。研究結(jié)果表明：在4℃下保存6d的血清不適合進行Ig的檢測。但是在-20℃下保存的血清卻可以進行Ig的檢測。
